HOWARD — Horizon Health Care, Inc. (Horizon) has entered a partnership with TriWest Healthcare Alliance to offer the Department of Veterans Affairs (VA) Community Care Program in all of Horizon’s medical clinic locations.
Through this program, each Horizon medical clinic will be a VA Authorized Community Care and Urgent Care Provider. The purpose of this program is to enhance access to health care services for Veterans by making a quality provider network available closer to a Veteran’s home. Access to care through this program is only available to eligible Veterans following a referral from a VA Medical Center that is processed by TriWest.
If an individual qualifies for VA health care, they are eligible to be authorized for coverage for the services needed in the Horizon clinics, including primary and preventative care, and behavioral health care.

No one should face barriers to accessing health care because they live in a rural community, especially not those who have served our country.”
